Which is the most influencing factor w.r.t. Quality Assurance?

I created following poll few days back, just wanted to see what people thinks about it. Though the poll is relatively new, I have a clear split of view.
Q: Which is the most influencing factor w.r.t. Quality Assurance:
-Testing method and test coverage (0 votes)
- Process and templates (3 votes)
- Requirement Analysis (0 votes)
- Customer acceptance (4 votes)

Just wanted to know what experts thinks about it. Post your comment, I hope it would be an interesting discussion.

Current status of poll makes it more interesting.
People have entirely mixed up opinion.

Your question addresses Quality Assurance specifically. QA is a subset of Quality Management as is Quality Control.

QA, as defined by ISO 9000 is: "part of quality management focused on providing confidence that quality requirements will be fulfilled". Essentially QA is a verification/confirmation that the Quality Management program is effective thus will achieve customer acceptance. The project Quality Management program would include the QM of all contributors to the project. The final QA then is a validation that all QM programs are appropriate and are being implemented effectively. This is primarily achieved through process and templates

Hello Sir,

Not deviating from the question, if its just an option, i would like to go with "Customer acceptance" - as the parameter quality is rated by customer/end user.

My bad, I missed it in between sentences, I was expecting that option selection will be the start or will be marked to identify clearly.
I read through again and now I got it. The miss was from my side :)

My bad, I missed it in between sentences, I was expecting that option selection will be the start or will be marked to identify clearly.

On technology projects are they engaged and interested in the development of test cases? When the answer is no, I suspect that quality will be in trouble, but they expect you to resolve it.
