A project manger in a global organization is responsible for a project involving six different companies. Midway through the project, one stakeholder's involvement is lessened due to illness. What should the project manager update?
Aditi came back again ignoring request to put these queries to other discussion group. :D
Agin sorry to say this doesn't look to be good drafted question. Saving Changes...

Aditi,
I would guess
a) Stakeholder engagement plan as it contains actions to ensure appropriate involvements of stakeholders (like meeting attendance, response times, expected attitudes).
My second candidate is c), but it does normally not include more than contact data and is shared within the wider team. The personal situation of one stakeholder should not be shared with the public.
b) is never updated (I know some dispute this, but at least it ois correct within the PMBoK context)
d) includes a) but a) is more specific and there is no other listed component of d) that would contain such information. (Maybe the issue list).

I will also go for “a“ incase of need to select out of given options. But the question do not include much clarity to select “a“ as well. The given information do not call for change in stakeholder engagement plan either.

True, like many questions this is quite unclear.
It may be meant to show a deep understanding of the concepts and chose the answer that makes the most sense, eliminating those that don't and comparing specific ones vs. general (like a and d).
If that is a mockup question, I would say it is great, as it forces you to re-read the definitions and purposes and with that gain a deeper understanding.
If that is a real exam question, there are easy and hard questions to spread the results of exam takers.

Here I might differ in opinion.
In my view if the question you need to think which is best, as all options looks correct than it is a good question.
But if the question make you to think which one should I choose as all looks wrong, than it is not a good question.
Here and last few from Aditi were missing clarity in question and also options given are not fitting to possible good choice.
This should not be a question in exam.
