Interesting questions but all leading to the same source.... you start with the business plan, the organization's strategic direction and ask one fundamental question... Why can't we do this today? In the answer lays the GAP and in that GAP the metrics needed to measure improvements.

I am answering the below question.
How to start Measurement Gap Requirements and Reports
Step 1: Find out the entire initial requirement.
Step 2: Decompose requirements into the small requirement which cam be splitted into 8 hr/ 16 hr structure.
Step 3: Find the requirement in Later stage
- The hidden requirements which you and your developer found after requirement phase
- The requirement was misinterpreted in initial stage and you understood those in later stage
- The requirement can be changed to something better which you identified in Later stage etc etc.........
Step 4: Find all the GAPs like the above and define time against all the individual activity lekage.
Step 5: Then Map the lekage requirement with initial requirement. And get the percentage.
Note: Do not jump directly to any rule defined. Remeber Metric will not help anything , if there is no proper action/process taken.
