Does anyone know if PMI has any standard templates for project approval or do most organizations make their own?

There are no standard templates for any PM deliverables/artifacts. Organizational policies/standards and the context & needs of a specific project will dictate this.

In line with @Kiron, we use a stage gate process to keep project governance which is totally independent of the life cycle we use. Into each stage gate we use a related presentation to request approval.
