Template/Checklist for transitioning new platform/business support processes to another group

I know, sounds like a lot - but I can't seem to find something that would help me out. THere are some templates on here but nothing I would call inclusive.
What I'm looking for.
1) A transition plan (which I believe I found)
2) What all should be considered (i.e. normal support - like help desk, tier 1, tier 2, ongoing training, patches/matintenace, roles and responsibilities, etc....
