Hi Alok,
As per my experience improvement of any process lies in hands of there users i.e Employee of the organization. Therefore, have a procedure for there feedback about existing process and improvement suggestion. Also, they can provide suggestion for new process as an additive which will provide benefits like value addition, ease of use, cost cutting, removal of redundant procedures etc.
One last thing is attach this with some reward or KRA to get real benefits.

I agree with what Rakesh said...
Besides that a Process is meant to facilitate the execution of project.
During execution we might come across things where following a process might hinder the execution.
In such situation we must allow flexibility in our process or redefine our process to meet the expectation of time.
But in any situation we must record this fact why we needed to divert from the process and what benefit we achieved...we can also record how a process helped in a given situation.
The input and output of the above mentioned step can be used in Process improvement program.

Six Sigma is a process improvement approach (although there are others). It essentially looks at cutting out non-value add steps in the process. It is often interesting to see how much people understand about the impact of what they do on others later in the chain. If they don't know that by doing something in a certain way it creates problems for people later, they will just carry on doing it that way. Bring the people who work within the process together for a discussion, and make sure the process has an owner. Saving Changes...
