Why Agile Projects Fail And What To Do About.

Organizations fail in agile projects because:
Organizations who set out to establish agile methodologies without providing teams with a clear understanding of WHY.
Stakeholders who went through failed agile implementation cringed when someone suggests let’s try an Agile way. And will now be cheerleaders to squash the new implementation of agile.

Agile for agile sakes is not a proper goal of an organization.
What makes an organization truly agile isn’t the adoption of processes and standard routines but understanding why those processes are in place and what are the principles behind them and the impact they’ll have if not properly used.

Just like Values Stream Mapping (VSM - LEAN) of material and information flow diagramming from LEAN methodology that analyze the current state and designing a future state for the series of events that take a product or service from the beginning of the specific process until it reaches the customer. So should the organization adapt Value Stream Management Mapping (VSM - Agile). VSM - Agile is getting ideas through development and into production with the combination of people, process, and technology that maps, optimizes, visualizes, measures, and governs business value flow (ie. in the form of epics, stories, through tasks or work items) through enterprise delivery pipelines.
Organizations who went through successful agile transformation if they are honest will tell you that sometimes their agile teams thought about giving up. They tell you that you’ll have good days and bad days. Just like building muscles which don’t develop over night or over a sprint or several sprints. Stick to it throughout the journey.

Agile projects fail because Agile project do not exist. That´s the point. So, if somebody call something "Agile project" then is DOA (Dead On Arrive).
