Would be good to hear what your good and bad experiences have been with implementing Scaled Agile.

Are you referring to SAFe specifically or any type of scaled agile approach?

The clients I've worked with have struggled with scaling agile mostly because:

1. They have difficulty being agile with "small scale" situations
2. They are unwilling to overcome organizational impediments to greater agility
3. They think agile is a noun and not an adjective

One of the challenges with SAFe is that even though there are a few different configurations, it is a revolution introducing it within a division or company rather than an evolution and hence there is a stronger likelihood of change resistance.

There is also a greater risk (due to its greater complexity) of teams "doing" SAFe rather than embracing the principles on which it was built.

There is an old and often ignored opinion that Agile (capital 'A') is a cultural phenomenon based on values and principles rather than organization charts. Therefore, it cannot be scaled. An organization can merely adopt or reject agility.
