"Write a Business Plan" - help!

I have been asked to assist in writing a business plan for one of my projects. There is already an ROI, and the project is approved and about to start. Management wants something describing milestones, indicators, etc. that will be monitored to ensure that the project is doing what it is supposed to. It is an enterprise-wide initiative, and the person working on the business plan with me is the GM. She hasn't written a business plan either, and we're hoping to find a template or example to work from.

Any suggestions?
