Knowledege transfer process from a PM

In IT industry changes take place at very quick pace and this requires a hell lot of knowledge transfers at individual and team level. Please share your ideas to make this process streamlined and managed smoothly. Please share if any tools or practices available for this?

Alok, do you have a corporate or project wiki? That's a good place to store knowledge as a project progresses, in order to facilitate a handover.

I think the best approach is to 'design in' knowledge transfer as you go - make sure all the documents produced are in a format that someone else could pick up easily, that kind of thing.

Alok, There are lot of process devised at Organization basis for knowledge Transfer at all levels . It depends on the type of Organization someone is working for and standard/process followed there.
Elizabeth is right stating the use of Wiki or may be share point is also good place for sharing information.

Also , one important point is apart from tools and process it is always crucial to have face to face discussion for handover/takeover with outgoing person and define or devise the steps in mutual understanding with each other.

Hand over should start at the very beginning of the project. Include the people who will be supporting or using the project deliverables in the project. Use them in workshops, testing and training. In the process they will also become your change champions and break down that "us and them" mentality. By taking them on the journey you are not effectively throwing the project deliverable "over the fence" at the end of the project.

Julie has answered your question, totally agree with her that must include all the interested parties at the beginning of the project, it is no longer about IT delivering and walking away once the project is completed. It is about the end user [the business] external support [vendors\suppliers SLA's] etc. they all need to be part of the project from start to finish.
