I am at a bit of a loss here on how to answer. Did the end date creep up on you and get you by surprise? You really need to be more pro-active in project management and use your schedule and actual work outstanding to show that you are not meeting your dates and do something about it as soon as it starts to slip.
Your regular status reports should have been red for some time and a change request to re-baseline or get extra resources and new project plan submitted to your sponsors.
At this late stage you need to work out with your team how long it will take to complete the project and discuss options with your sponsor. Do you need more resources, more money and/or more time and how much of each. You need to identify why you are running late and address that as well because there is no point in replanning if the same thing is going to happen to the new plan.
You could use an Exception Report! state the fact that the project has missed the delivery date, the impact it is likely to have on budget, additional costs and resources and duration to finish project. The Exception Report must be discussed with sponsors\stakeholders for them to agree the new plan.
I am a little surprised that this has happened, did anyone else notice the project was not delivered on time and within budget!

I agree with Vasoula, an Exception Report is a good document to use. Alternatively, if the project closure date has passed and this was a fixed date, missing it may well mean the project is no longer necessary. In that case, you could close it down normally, and a project that did not deliver the expected deliverables or benefit.

Let's say some problems happened during the excution of the project and the project could be halted for some period of time, what kind of formal reports should be sending out to the management?
As for me, i just send email around to the concern people and telling them what are happenning, why, when we expect to start it again.
However, i just feel that it is not the way, and i am looking for the better one.
