Looking for background/insight on Citizen Developer (aka Citizen Developer 101)

I cannot fathom how this can work in so many regulated sectors. Who is managing the work product? Where is the initial code coming from? I have tried researching but all I have found on this assume that I have prior knowledge. Thanks for any information!
