when a sponsor included in CCB after approving of a change request ,and while start implementing the sponsor change his mind and want to cancel the change request what should PM do?

i think we should apply change request to cancel the approved change request

As you stated a CCB or the way somebody can call it is not a person only. Project manager duty is to manage all activities needed to create the needed information for CCB decide. Everything related to changes have to be presented to them. Why? Because they have more information than the project manager mainly related to impacts in other initiatives which are running at the same time. So, actions on changes must be decided by the whole CCB, the PM or the Sponsor must not decide in isolation. Final remark: all this has sense if the project change management process shows that. If the project management process say other thing then follow it, so simple than that.

once a change request has been approved and baselines are updated, that is the new basis for performance measurement and change control. If there is a desire to roll back the change, then a change request should be issued, reviewed & approved.

How formal the process is, and how much work will go into the analysis and implementation rework is context-specific.

Just to add a little bit to the excellent input of my colleagues, the change request and approval is required to ensure reversing the previously approved change is fully integrated.

It may be as easy as telling one team to update their backlog to remove accepted work that was never started. The change could also require horizontal integration across multiple design teams, and vertical integration through the entire supply chain. That could impact contracts, purchase orders, staffing, financial planning etc.

The sponsor may have sole authority to approve the change, but one of the purposes of the CCB is to ensure all that is captured, and you are backing out the entire change, and not just the prime group impacted.

-Follow the Project management plan.
Before what mentioned above consult yourself in the the nature and consequences of the change required.

CCB - Change Control Board? If you want wide commentary on your question please consider defining your acronyms. Could also mean Community Church of Boston :-)

Sounds like someone is making decisions without a proper business case. The solution is arrived at before the problem is defined. There have been three solutions pursued so far and I'm not convinces its the last. 1) solution A; 2) solution B; and 3) back to solution A. Has the problem changed or was it poorly defined? Was the analysis questionable? What makes us think that the latest twist is the last?

The PM should drill down and determined what the real problem is. Alternatively, get ready for significant project disruption and most likely project failure - cost, time and product suitability are all on the line.

What Kiron says above is what's right, but it doesn't mean that there won't be political blowback. Most leaders will appreciate you honoring and being accountable to the agreed to process. Others may see it as a loyalty or respect issue. I've been on both sides of this.
