Which document describes the acceptable standards for accepting a change request?

The Project manager observes that team members are accepting a number of change to the deliverables with little attention to ensuring acceptable standards. When questioned, the team members reply the changes are relatively minor. What part of the project management plan should the project manger review with the team?

a) Perform Integrated Change Control Process
b) Change management plan
c) Scope management plan
d) Quality management plan

this is an 'additional component' of the project management plan.

It probably will contain the project specific process A, which is not mentioned though to be a part of the project management plan.

I had also chosen "B" but the practice tool displayed my choice wrong and gave the following explanation of a correct response. This explanation seems to indicate that vendor's choice is (d) "Quality Management Plan". I wonder why?

Practice Tool's explanation:

Quality Management Plan describes how applicable policies , procedures, and guidelines will be implemented to achieve the quality objectives.

Definitly b). The Change Management Plan should have an explaination as to why there is such a plan and when it applies. It seems from the question that the team understands what changes are but do not recognize the impact of changes.

I find this to be a problem during the design phase of infrastructure projects where designers and stakeholders make a multitude of 'minor' changes not recognizing the cummulative impact on cost, schedule and quality. There is also the precidence issue - once minor changes are part of the culture, it becomes harder and harder to manage the scope.
