As far as applying to PMI to take the PMP certification, can someone tell me what the following means?
... possess documented evidence of your experience in directing and managing project activities ...
The vast majority of my project management assignments were from companies where I no longer work. Due to the (state?) law, they cannot give any information about a past employee other than yes they worked here and start and end dates.
What “documented evidence” will I need to supply?

I recently applied for (tested and received) my PMP certification. Granted all of my experience has been with one employer the evidence that I provided as part of the application process was related to individual projects and phases within those projects. For example, I laid out how I directed and managed a project through the Process Groups. I did not have to supply direct evidence such as a project plan or project schedule.
However, my application was audited and I DID have to get verification of (via a signature) of the project experience that I listed from my manager (VP of PMO). I would suspect that if you were similarly audited that they would ask for verification of project work from a previous employer if those hours were being counted towards your PMP credentials.
