Critical Factor Transform to Agile Mindset?

Can you share, whats is factor (critical) that most organization failed to transform to Agile Mindset?
Its Age of the organization one of the factor?

Steve Denning wrote an informative article in Forbes about the agile mindset.

Among other things, like debunking the validity of the 'growth mindset' as described by Carol Dweck in 2006, he saw roots of this agile mindset in the Theory X and Y views of managers about staff. This is based on a 1960 book by Mc Gregor and has for long been an often asked topic in the PMP exam.

Interestingly, he still calls the 4 priorities (X over Y) in the agile manifesto 'values', while they are just beliefs, dogmas. 'We have come to value' does not say that the following statements are values and therefor inherent truths, but it rather says that some experience of a few people made them jointly belief in those statements. Typical setup of a sect. The rest is rationalising.
