What does you mean with requirements templates? Because if you are searching for a document to specify requirements you will find a lot and they will differ according to the type of requirements you like to state, the method you use for stating them and the degree of formality.
For example, what I am using for years if a degree of formality is needed are IEEE standards like:
IEEE 1233-System Requirements Specification where system is not software system, is the whole organization from architectural view
IEEE 1362I-Concept of Operation for business requirements
IEEE 830-For Functional and Non-Functional Requirements
and so on, where each one is derivate from the previous one.
While them are intended for software in some cases I an using them for any type of solution, software or non software.
But if I do not need a degree of formality I could use a simple card to write user stories just in case we agreed to use it and then use case templates to clarify each user storie.
And I can write about other ways I use and I used depending on the environment and mainly the method to interact with my stakeholders.
So, the template you will use depends on the degree of formality and the method/technique you are using to specify requirements.

I do agree with Sergio. You need to be more specific.
