I think you should develop yours based on your project-specific circumstances.
The factors you've listed are likely relevant to any project so even a generic project assessment checklist (I'm sure there are some in the templates section of this community) will be a good start, but you'd then want to add other measures related to the specific nature of your project - e.g. compliance with medical industry regulations, health & safety, expected vs. achieved benefits and so on.
IBM has an assessment framework that works for any project. You can google it or search form it on slideshare.
It is called 7 keys to success.
