The latter (you are teaching them how to fish...) but as usual there are exceptions where the leader might need to actively step in to eliminate an impediment.

Both, depending on the situation.
It is a balance looking at people development and project progress. People development needs a long view and requires to maintain trust.
Project progress can take a hit, if you are confident, the people and you can recover. Again a matter of trust.

I think the definition to be a servant leadership is "to internalize" that you are doing well if the others are doing well, where it does mean that you can help people to improve theirself and advance in their life.

Leadership--whether servant, collaborative or otherwise--is about providing people with a safe and supportive environment for them to learn and grow. Removing the obstacles yourself addresses the first part but not the second part.
Sometimes we talk about "protecting the team". We have to be careful that we do not become mother-hens, shielding our "children" from the real world.
