How frequently do you revisit risk information in your risk register?

Dr. David Hillson has defined risk as "uncertainty that matters". If your stakeholders are ignoring your risk response recommendations it might be that how risks are positioned doesn't matter to them.

One of the ways this can happen is if risk information is out of date.
