When I worked with TD Bank for a few years, I don't recall any program which had separate project steering committees as well as a program-level one. As a steering committee is usually made up of executives, such a structure would impose a lot of overhead and be pretty difficult to staff effectively.
I've also found that the term Steering Committee itself means very different things in different companies. In some instances, it is an advisory group which helps the sponsor and/or PM make decisions whereas in other cases it is a decision-making body and a point of escalation for the sponsor and/or PM.

If you read through the PgM guide for PMI, it is defined there. The Program Steering committee is a bit different than that of Project Steeering Committee in terms of members, the items you review, and so on.
As for your second question, the answer is yes. Component projects have separate committees that fall under our program in addition to the program steering committe.

while a project SC focuses on the parameters set out in the charter, like schedule, scope, cost and wants to see that risks are handled, a program SC focused on benefits as laid out on in the business case and wants to see opportunities.
There is governance within a program and a program manager will support component PMs in many ways on daily base anyhow, so the need for a special projects SC is often not given.
