Recommendations for Project Team Performance Appraisal Criteria

I am developing what I call a 360 degree project team performance appraisal where at the end of the functional managers will receive a performance appraisal from me as the PM on how well the person performed on the project. Additionally, each team member will be provided the opportunity to submit an anonymous google form survey providing me feedback on how well I did as the PM. I haven't been able to come up with objective values yet for appraisal criteria 1 and 4-7.

I'm looking for others feedback on if you do team performance appraisals, how you do them and criteria used.

Below are some of the appraisal categories I am considering and each area will have a rating scale of poor, fair, good, Excellent, or N/A

1. Quality of work performed on project
(Determined based on results of testing, and any defects found)

2. Timeliness of project work completion
(Comparison of actual work duration and elapsed duration. If Work Efficiency is 75% or higher=Excellent, 50%-74%=Good, Less than 50%=Fair)

3. Contribution to meeting project and budget targets (Determined based on % of Tasks completed at or below estimated work. If 75% or higher tasks completed within planned finish date, rating of Excellent, 50-75% tasks completed within planned finish rating is fair-good. Less than 50% of tasks completed within planned finish, rating is poor-fair. )

4. Cooperation with other team members
5. Communication of progress, status, issues
6. Communication on technical matters
7. Contribution to continuous improvement of development, technical, team processes

Sounds good. However, organizational culture matters.
