Does anyone have a template for a Service Catalog?

I'm looking for a template for creating a Service Catalog. I could easily create something, but I'm wondering if someone has one that works well, gathers the right information and is either used as the published document to the business, or is the input to a published document.

The ITIL framework may have a service catalogue framework that you could use. Have you tried talking to your IT Service Delivery team?
