How do you initiate a programme, any template or guidelines available?

Check the Templates section of this community, but in general before a program is initiated, a program business case would have been produced by the sponsoring area and once that business case has been reviewed and approved, a program charter would be developed and signed off.

Of course, this is general guidance - organizational project standards and policies might specify a different approach.
