Please, does anyone have the complete directory of Opm3 capabilities?

In the OPM3 (knowledge foundation) in appendix G it indicates that the capabilities directory is on a CD-ROM, but I couldn´t found it.
The directory must contain all organizational capabilities, with their respective definition and relationship with good practices and results.

Does PMI have an alternative replacement for OPM3? Can OPM3 still be used to conduct Program assessments? I'm stuck on the word "discontinued".
