One "how" is a PM's use of a toolkit such as Disciplined Agile to help them better understand the context of a project so as to help their team define a good starting point for their "ways of working".
I start from a basic framework, and then customize it to better fit my needs, but then the trick is you have to keep adapting it.
I try to avoid organizations where there is one process and/or tool to do everything. Those turn into One size fits none situations. I start with a basic framework using DA as Kiron suggests or my own experience. Then I consider where that framework is a poor fit and start modifying with ideas from other frameworks or tools I know.
A major mistake is not recognizing when the situation has changed and the once good approach is no longer working, or not working for all parts of the project team. Just like a method can be revised over time, the same method isn't always necessary for all parts of a project. Saving Changes...
