DAIR Template

Hi everyone

Recently I've seen references to a DAIR register, an integrated register for Decions, Actions, Issues and Risks. However, despite my best efforts, I cannot seem to find a template that I could use. Does anyone know of one?

I don't have a template to send you, but it is pretty easy to make your own. I wrote a series about RAID (as we call them) logs for PMTips in 2011 and included screenshots. Here's the link to the Action section, you can see how the log is laid out - it's just a spreadsheet: http://pmtips.net/raid-logs-actions-element/
Make each of the elements a different worksheet in the workbook and you can pretty much use the same log layout for all the worksheets.
