looking for a checklist on project cancellation

The vendor was unable to meet deliverables in this project, so a decision was taken to terminate the contract and identify another vendor.
Should the project itself be cancelled or focus just on replacing the vendor but keep the same name of the project.

Would also like to get a checklist and/or some information in this area?

Until your company is willing to accept its own contribution to the vendor's failure to meet deliverables, just replacing the vendor will likely lead to the same result.

Whether or not to cancel depends on the project objectives - if they could be met by using a different vendor then it doesn't need to be cancelled. But if they objectives were tangibly focussed on one provider e.g. "roll out MS Office 2013" & MS have said it won't be available in your timeline then I'd cancel.

As for a checklist- as a start you should go though the whole project plan & identify everything that needs to undone, and add some communications deliverables.

It sounds like you are still trying to deliver the original goal, just with a different vendor. In this case, I would continue the project, but would recommend a rebaseline, as original cost and schedule are probably blown at this point.

Unless your objectives are fundamentally changing or the business case is considerably different, I wouldn't cancel the project. Just change the vendor. Although, as Wayne said, it would definitely be worth rebaselining. You could put the project on hold while you go through the procurement exercise and then reinstate it as an active project when you have found a new vendor.

I haven't come across a checklist for cancelling projects but if you find one, let us know!
