If you're having trouble with the application tool itself on PMI.com, then I'd suggest contacting customer care. I tried to open up the tool to see what it requires for input but get an error because I'm already a PMP. I don't remember uploading any proof of completion for the university course I used in my application but that was several years ago. Saving Changes...
