On my driver's license, it says last name first name middle name. When I take the CAPM does my name need to be listed in that format since the driver's license uses that format? Or is just my first name middle name last name ok? Saving Changes...

As far as I know, when you fill in your application, it will say First Name and Last Name so input your First Name in the first name slot, and family name in the last name slot. In my driving license, it is somehow listed as yours and never was an issue. That said, this is my personal opinion and experience, so I would contact PMI Customer Care for reassurance, like Kiron suggested. Good Luck! Saving Changes...
