Hi Mohammed,
I have been a PM for many year and have never heard of a scope management plan. The Project Management Plan should contain the details or references to, change management which identifies how change is managed of which scope is just one aspect. Do not get mixed between change control (Process for approving change to project scope, cost, schedule etc) and organisational change management which addresses the human side of change, training, communications, change champions etc.

Dear Julie
Thanks for help, however the scope management plan defines the procedure you will follow to manage your scope ( collect requirement, define scope, WBS, validate scope and control scope) and change control management should be part of it. the target is to make a deliverable 100% complete and conform.

Hi Shikha...I did find one out there on the Internet. I used the words "scope management plan template" in Google and items 5 and 6 were very helpful. One from the state of TN and another from George Mason University (gunston.gmu). I hope this helps.
