How wouold you structure your governance, around waterfall and Agile pieces,of the Blended Approach?

Many of you must have had experience in Blended or Fusion approach - using Agile Practices with waterfall processes, in the same project. How do you think the project governance be structured to unleash the benefits of such approach?

I may, possibly, depend of few Organizational Variables as well like - 1. Risk appetite/threshold; 2. Environment of the Organizations - Projectized/Matrix etc; 3. Pool of Project Managers - Generalist vs Specialists etc
