Meeting Minutes question

We have weekly meetings so a lot of topics from unfinished business come up, we discuss any progress that has been made, but they stay in unfinished business until the issue is resolved. The question I have is when writing new minutes, and I am referencing an ongoing issue that is discussed but will stay in unfinished business, are there guidelines for citations? Some in our meeting believe we should put a citation to when this topic came up in new business and others think we should put a citation back to the previous minutes when the issue was updated. Due to the frequency of our meetings, there are numerous items that we keep in unfinished business from week to week.
