Knowledge transfer is like passing the baton in a relay race—if done well, the project keeps moving smoothly. A practical approach is to have team members shadow their replacements, allowing knowledge to be shared naturally through hands-on experience. Keeping documentation simple and accessible on cloud platforms ensures that critical information is easy to find. Short video walkthroughs can be more engaging than long documents, helping new members learn quickly. Regular knowledge-sharing sessions encourage teams to discuss lessons learned and avoid repeated mistakes. Simple checklists and structured processes also ensure consistency. The key is to make knowledge sharing a habit, not a one-time task. When teams actively document and share insights, projects remain strong even through transitions.

In addition to many of the items Pavan listed I keep a log detailing the reason certain actions were taken and decisions made. I have taken over projects from other Project Managers many times and I have often longed for a historic record explaining these, as most people involved with the project won’ t recall this information, especially if the project is complex and/or of long duration.
