Many of us document lessons learned at the end of a project, but they often end up buried in folders or forgotten over time. In the past we had built a Wiki Page using Confluence for capturing the Lessons Learned. My Team would go to this if they face any issue (technical or process related) during the project execution. Lessons Learned Register was outcome of our Retrospective meetings.
