In that case it would be Define Activities, but because that process updates components of the PM Plan, I'd argue that those are also "Project documents" - this is a long standing issue I have with multiple editions of the PMBOK Guide that haven't come up with a better term than "Project document updates" to cover the "other" documents.

I completely agree that, in practice, almost any process can generate insights that deserve to be captured in documents like the Assumptions Log or the Risk Register.
However, for exam purposes, we go by what is explicitly listed as outputs in the PMBOK Guide.
The distinction between 'Project Documents' and components of the 'Project Management Plan' does create ambiguity — and it remains a poorly resolved gray area across multiple editions of the Guide.

Yahya, in principle, all those could generate changes to project documents but if I have to chose one, I will chose option C “Sequence Activities” because this activity primarily results in updates to the project schedule network diagram and may update project data or outputs, but it typically does not generate changes to project documents.

I have found a lot of the practice exam questions that I consider fundamentally flawed. A-D are all part of the Planning Process Group. From Process Groups: A Practice Guide p.77:
A) Estimate Activity Resources (5.16)
B) Estimate Activity Durations (5.9)
C) Sequence Activities (5.8)
D) Define Activities (5.7)
All of those are documented, and not even the wording was changed.
I have found that due to the poor quality of many of the practice questions, there is more value reasoning your way through why it's a bad question than getting an answer that the author of the question intended.

I`m confused why PMI agree to this, wasting of time for no reason.
Alot of questions have no sense and when i complain to the trainer he says ' IM ATP of PMI and these questions are approved from PMI!!
In general, the real PMI exams will not ask such "straight out of the book" questions. The trend has been to focus more on scenario or application oriented questions.
