Project Charter is created for the entire project once, but not for the phases of the project. It always be one Project Charter irrespective of phases and iteration of a project phases.

at times, projects do have multi phases or even overlapping phases. In all cases we always create single project charter and project mgmt plan. These may get updated at later stage, but initiated once at the beginning. Hope this helps. Saving Changes...
Firstly, one should understand why this phased approach is opted for?
Is it due to financial constraint? or uncertainty with respect to scope?
Further, when this project is done in phases, how is the project business value is evaluated? for each phase? or as a whole?
Does the project deliver certain business value at the end of each phase?
If yes, the economics of the project will be evaluated for that phase?
Will the organization make the final investment decision for each phase?
Such issues are to be addressed at portfolio or program level.
Is there a time gap between initiation of phases?
Does the end of phase 1 is linked to end of phase of another project or program?
Considering all the above, organization can take a decision to execute each phase separately and depending on the product, service or result for each phase, the charter and other such documents will need to be prepared. Saving Changes...

Project Charter is the first in 47processes. The Charter typically contains high level inputs from business case elements, risk, schedule, budget, project team-stakeholders..etc.
They may vary based on industries, hence please sync with internal process requirements.
