How did you secure agreement to have a Business Owner in health care projects?

I thought this article (http://www.healthcare-informatics.com/arti...wner-inclusion) was a great start to addressing your question. The author relates a situation where the project had lost focus, and how re-involvement of business owners was part of getting the project back on track.
One concern that the article doesn''t mention is when the business owners make poor decisions due to their personal agendas ("but my friend is selling this piece of software so I want to go with it"), rather than giving more weight to the advise of domain experts who have done their due diligence. This can produce less-than-perfect outcomes. So while it is imperative that business owners be involved, the Project Manager has to make sure to *manage* to project objectives--sometimes difficult for sure.

Typically the clinical department or clinical leadership backing the health care project will identify a business owner (or clinical champion) to provide oversight for the project. It has not been my experience that project teams need to secure agreement to have a Business Owner assigned.
The Healthcare Informatics article that Bruce Wilkinson shared is good and spells out many of the duties of the business owner. I would not expect the business owner to get too much into the project's details (e.g. backlog definition, securing resources, etc.), but certainly they would provide key input during the various project phases.
