What is the best way to deliver applications in a 24/7 worksite to ensure that there is minimal interruptions?

There is no secret formula = it is very difficult. Contingency planning is my best advice.

For application delivery to be efficient and uninterrupted I can think of few points as below.

1) Contingency planning (for each and every asset required for application delivery) as said by Kevin.
2) Competent workforce and their backups
3) functional and technical plans
4) allied management plans(security, admin, transport, employee well being,etc)
5) resourceful leader
6) etc
7) etc

My keyword for that is JUST-IN-TIME (JIT). Ensure everything is surgically and exactly connected in a critical chain with absolutely no wastage and delay. A highly efficient lean system.

I agree with Saurabh. He has given a good measure of it. Observing best practices of ITIL and some lean principles will help things better.
