You fix an issue.
It gets:
- Tracked
- Discussed
- Escalated
- Resolved
Everyone moves on.
A few weeks later—
It’s back.
- Different project
- Different team
- Different name
Same pattern.
- Missed dependencies
- Late escalations
- Unclear ownership
- Shifting priorities
Again.
At some point, it stops being a project problem.
It becomes a system problem.
But most of the time, we don’t treat it that way.
We:
- fix it locally
- document it as a lesson learned
- move on to the next delivery
And then act surprised when it shows up again.
It makes me wonder:
Are we actually solving problems…
Or just getting better at managing their recurrence?
