Who is really on the hook for CRM Failures?

CRM implementations been failing for years. Their troubles stem from their first inception. Opposed to what the vendors say, these implementations are not about configuring software, they are about culture. You might be absolutely right depending on that fact where the bottleneck exists. If CEO office is the bottleneck, they would be responsible for all CRM failures.
