you can find lots of templates if you search in the internet. . Just choose the one that fits your needs better. You could also consider using tools like Agilefant, Pivotal Tracker etc

Is there any particular reason you feel you need a template in the first place, Javonnia? Is it necessary on your project, for example, to facilitate sharing of the backlog in electronic form? Otherwise, I''d suggest trying plain old index cards or Post-Its, and keeping the format very simple.

If you do go to an electronic method, try to choose a simpler tool at first, before trying to work with one of the heavier, more fully featured agile project management tools.
