On projects of an industrial nature, what types of risks have caused the most scope creep, schedule delays and cost overruns

Projects of an industrial nature often have scope creep, schedule delays and associated cost overruns. I have some thoughts on what risks cause the most upsets, but I''d like to hear from others to validate or invalidate my thoughts. Also, in addition to the types of risks you can identify, would you say they tend to occur in a particular phase versus another? Which one?
