Bernie, I can't recommend a tool but can offer some suggestions about what the survey should measure.
1. Did the project deliver what was promised?
2. Was the process followed suitable and painless?
3. Were the results achieved worth the effort?
4. Would you seek to use the project team again in the future?
This four way view will provide excellent feedback for continuous improvement in future projects.
