If I understood your question correctly, you want to conduct a project level audit, a project that is following Agile, if so - you may want to start with study of project plan document that shall give you inputs about how and to what degree is project expected to follow Agile ceremonies and practices as they may vary, and come up with audit check list which shall be your tool for audit.
Also you may want to consider reviewing organization and team maturity in Agile when you audit a project.

I read the question differently than Ashok. Dimitra makes no mention of any particular project, but is trying to make an assessment at the organizational level.
Dimitra, I would question why you are seeking to perform such an audit in the first place. Who is going to consume such a report? Are your teams not able to identify areas for improvement themselves, in their retrospectives and other interactions? Aren''t you already aware of the major problem areas in your organization? I look for an assessment on organization level, not a specific project. The reason is that we want to evaluate the level of agile adoption and identify potential areas for improvement.
Teams are identifying certain topics but we want to assess the bigger picture.

What metrics or measures are your teams capturing? Do you capture and share lessons learned from retrospectives across the organizations? Is there a PMO that helps coordinate? Have you done any feedback from product owners and business area stakeholders? How do they feel about your use of Agile? What has been the overall performance of the Agile teams?
