well, i am personal have used the mentioned double by Rita''s and Head First and it worked for me, so i would recoemmend those ones, but they havn''t ot be the best choice for you. You have to do some research to figure out which of the stdy books fits your preferences best.
I do not know a questioon book which fits your requests like mentioned in your post. If you are a PMI member you can access the question book by Scordo (in the didital library) but i havn''t use this one by myself.
My recommondation would be to use a professionell online exam simulator; i have used this one and it worked great for me:
