I do absolutly agree with Prolay,
ground rules - not only for meetings - are very important for the project team; they should be developed by the project team right in the beginning of the project and established not only in the project meetings but also through the project life cycle in general.
From my experience the most meetings are such waste of time because of missing agenda''s and cause of missing a defined meeting goal.
What is the expected result of the meeting. Shouldn''t be that difficult to define this. At least it should be defined in the beginning of the meeting, so it could be the first point on the agenda.
When i become a meeting invitation without agenda i do always ask for it. If there is no agenda mostly i decline my participation.

While it works better if you set the ground rules at the beginning, it''s never too late to capture and identify them.
My suggestion is to have the group decide on the ground rules. It''s easier to get the group to abide by the ground rules if the group set them up in the first place.
If you find yourself in a situation difficult to control, you can always refer back to what they agreed and, if necessary, ask if they need a new rule. Saving Changes...
Meetings have a fixed time frame. With in the available time frame its ideal to break the Topics into sub topics . For example :, to start with we need to have an Introduction and purpose of the meeting (Agenda). We should have a plan to stop people to go off the topic and bring them back into the topic. We also should encourage people to talk and contribute who are silent and have a resistance to open up. The Host should give a pause to stop people from side conversations. Meetings should be result oriented. There is no point in having an agenda, if you are only going to keep on discussing the items without deciding anything. For this, meetings of different types should not be mixed. Performance review meetings should never be mixed up with meetings for generating ideas or for finalizing the requirements.
Regarding wasting of time in meetings, in 5S quality management system, there is a concept of standing meeting, where the meeting is conducted while standing. This not only reduces the time, but also keeps people on their toes (pun intended). Saving Changes...
