Ground Rule for Meeting

For an effective meeting, we need to establish the ground rules. I am very surprise that meeting invitation, did not mention about the ground rules.
People just make meeting and try to focus on delivery. Did you experience or see things like that in company Do you think meeting are becoming so much of waste time because no ground rules?
