Transition should start with Current situation vs Desired Situation.
First, draft the requirement management plan, how you will gather / elicit & document requirement. This document will include the current & desired state of Staff level & qty.
Second, The start gathering / eliciting requirements which will transit the staff to desired level. This document should include the No of approved positions, level of education, Hiring policies as well as separation policies.
By now, you will have a high level idea of what you need to do to complete the transition. Please the WBS, Assing, sequence, priorities the work.
In my experience, Transition project fails because of unclear desired state. Please keep emphasis on that till the project completes. Saving Changes...

You will want to establish checklists for transitioning people unto (access, training, orientation) and off (evaluation, debriefing, access removal) your project.
You will also want to have a calendar to show when people come up and off the project. Make sure to submit access requests sufficiently early before people are scheduled to start. Saving Changes...
