How to prepare a map of internal processes?

I need to describe internal processes performed by my department. I especially like the simplicity of ITTO format used in PMBOK (Inputs, Tools and Techniques, Outputs) so I thought of using the same approach for my processes.

Now I am looking for a tool to do that... and cannot find any. My requirements are quite simple: I need to be able to enter multiple processes, connect outputs with inputs and finally render chart with all the data.

Do you have any ideas?

From parctical experiene I recommend to use rule-based BPM-Tools. There are many. My preferred tools being used in practical project work (SAP Implementation Projects) are:
ARIS
Bizagi
TIBCO
All the three mentined tools should be embedded in a overall Business Process Management Strategy - meaning NOT perceived as a one-time documentation excercise.

Visio is very easy and helpful in mapping out current and future states. Use the Swimlane Diagram to make it clearer.
